A long-term study tracking more than 1,700 people for nearly a quarter-century has found a troubling link between heavy television viewing in midlife and brain changes associated with Alzheimer’s disease—particularly in men.

Published in Alzheimer's & Dementia: The Journal of the Alzheimer's Association, the research followed participants from their midlife years and later scanned their brains with MRI. The results show that men who regularly spent hours watching TV had smaller brain volumes in areas vulnerable to Alzheimer’s, more white matter damage linked to cognitive decline, and reduced frontal and occipital lobes—regions critical for decision-making, planning, and visual processing.

Interestingly, the same pattern did not appear in women. Lead author Natan Feter, Ph.D., told Science Alert that the gender difference may stem from men tending to watch television for longer, uninterrupted stretches, though hormonal and biological factors could also play a role.

Perhaps most surprising: people whose jobs required prolonged sitting did not show similar brain changes, suggesting that not all sedentary behavior is equally harmful. “Sedentary behavior has become a major part of modern life, but not all sedentary activities are the same,” Feter said. “Spending an hour reading a book is very different from spending an hour passively watching television.”

The findings held even after accounting for physical activity levels, meaning regular exercise did not erase the association between heavy TV watching and the observed brain damage. That challenges the common assumption that hitting the gym can fully offset the harms of a sedentary lifestyle.

While the study did not distinguish between types of programming, experts note that documentaries, educational shows, and complex dramas may be more mentally engaging than passive viewing. Anna Hohler, M.D., director of neurology for Northwell in Westchester, recommended using closed captions, discussing shows with others, and incorporating light exercise—like walking on a treadmill or using a stationary bike—while watching TV to make the activity more cognitively and physically active.

Researchers emphasized that television itself is not inherently harmful. The real risk, they say, comes when long stretches of passive screen time replace exercise, social interaction, learning, or other mentally stimulating activities that support long-term brain health. This aligns with broader concerns about cognitive decline linked to lifestyle factors, including diet.

For policymakers and public health officials, the study adds to a growing body of evidence that midlife habits—especially those involving prolonged passive screen time—may have lasting consequences for brain aging. As the population ages and Alzheimer’s rates climb, understanding these modifiable risk factors becomes increasingly urgent. The findings also echo recent research on blood tests that may predict Alzheimer's risk in healthy seniors, highlighting the need for early intervention.

Feter and his team caution that the study is observational and cannot prove causation, but the strength and duration of the data make a compelling case for rethinking how we spend our leisure time. In an era where streaming services dominate, the simple act of turning off the TV—or at least making it more active—could be a low-cost public health strategy.
